  • How do I register for an Ohio Medical Marijuana Card?

    1. Find A CTR Doctor

    Find and schedule a visit with a (CTR) certified to recommend medical marijuana doctor. For a list of all CTR doctors in Ohio, or to find a CTR doctor in your area, use the Ohio Medical Board CTR Search.

    The physician will need one of the following forms of ID: the patient’s valid Ohio driver’s license, a valid Ohio identification card issued by the Ohio bureau of motor vehicles (BMV), or a valid United States passport.

    The physician may also ask for medical records pertaining to the patient’s qualifying condition.

    Patients registered with a special status of Veteran or Indigent are eligible for discounts at dispensaries and reduced state registration fees. Patients wishing to apply for Veteran or Indigent status will need to bring additional paperwork to the physician appointment. See next FAQ “How do I register with Indigent or Veteran Status?”

    2. Complete MMCP Patient Registration

    Once your CTR doctor has entered your information into the patient or caregiver registry, you will receive an email with instructions on how to login to your patient registry account. Once logged in you will have to complete your patient or caregiver application and pay the annual registration fee of $50 for patients and $25 for caregivers

    Patients who qualify for Indigent or Veteran status may qualify to have their state registration fee reduced by 50%. Patients seeking to qualify for Veteran or Indigent Status cannot submit payment until their status has been approved by the Board of Pharmacy. The “Activate Card” button is not available while the status is pending.

    **Note for caregivers: While a patient’s card is active immediately upon submitting payment, a caregiver’s card may take up to 1 week to be active. We encourage caregivers to call the dispensary to verify their status prior to their first visit to a dispensary in the state.

    3. Visit Us!

    Once you have received your Patient or Caregiver Registry Card, you will be able to purchase at Southern Ohio Botanicals or any other dispensary licensed by the State of Ohio Board of Pharmacy.

  • How do I register with Indigent or Veteran status?

    Patients registering with Indigent or Veteran status may have their state registration fees reduced by 50%.

    To qualify for Indigent Status registration, the patient must be:

    • Enrolled in the federal Social Security Disability Income or the Supplemental Security Income disability programs, or;
    • Enrolled in a State Retirement System and receiving monthly disability benefits.
    At the patient’s initial CRT physician visit, please provide the awards letter from the current year showing the patient’s name and the amount of monthly disability benefits to be received by the patient.

    To qualify for Veteran Status, the patient will need to provide a copy of any of the following documents to the CTR physician at the initial visit:
    • Department of Defense Identification Card (active, retired, temporary disability retirement list) DD214, DD215, or National Guard Bureau military discharge certificate indicating disposition of discharge.
    • Report of separation from the National Archives National Personnel Records Center in St. Louis, Mo. Veterans Identification Card from the Department of Veterans Affairs.

    All acceptable proof documents, except a Veterans Identification Card, must show the veteran status as honorable, general, general under honorable conditions, or discharged or released under conditions other than dishonorable.

  • How do I find a Medical Marijuana Doctor?

    Many physicians are now accepting telemedicine appointments. For a list of all CTR doctors in Ohio, or to find a CTR doctor in your area, use the Ohio Medical Board CTR Search.

    The physician will need one of the following forms of ID: the patient’s valid Ohio driver’s license, a valid Ohio identification card issued by the Ohio bureau of motor vehicles (BMV), or a valid United States passport.

    The physician may also ask for medical records pertaining to the patient’s qualifying condition.

  • How do I register as a caregiver?

    Individuals already registered as a patient can be registered as a caregiver by being linked to another patient by their physician.

    The physician will find the patient to whom the individual already registered as a patient will be linked. Under the “Caregivers” tab, the physician will click the button labelled “ADD”. They will search for the individual already registered as a patient using the Registry’s search feature. When the desired individual’s name appears in the search results, the physician will click the button labeled “ADD TO PATIENT”. Once they click the button, the registrant will be registered as both a patient and a caregiver.
